Grants to Kansas and Missouri nonprofit organizations and government agencies in eligible locations for programs that improve access to quality healthcare for vulnerable populations. Applicants must submit a Letter of Intent prior to applying. Funding is intended to implement service delivery models for physical and oral health, which are safe, timely, effective, efficient, equitable, integrated, and affordable. Applicants can apply for project support, program support, or core support for ongoing operations.

Health Forward serves Kansas City Area, including portions of Clay and Platte counties; Cass, Lafayette, and Jackson counties in Missouri; and Allen, Johnson, and Wyandotte counties in Kansas.

Short-term program strategies should include access, quality, and costs. Long-term outcomes include better health, better care, and lower costs. For additional information related to strategies, see:

Project support is funding  for a specific, connected set of activities with a defined beginning and end date, explicit objectives/outcomes, and predetermined costs:

  • Projects must be new or significantly modified from ongoing operations and must align with the Safety Net request for proposal.
  • This funding category is also appropriate for organizations providing ongoing services that have been in place less than three years.
  • Allowable expenses include costs such as salaries, equipment, supplies, and other expenses directly related to the project.

Program support is funding for a specific, connected set of ongoing, long-term activities or services with explicit objectives/outcomes, and predetermined costs.

  • Allowable expenses include costs such as salaries, equipment, supplies, and other expenses directly related to the program.
  • This funding category is for organizations that provide ongoing services that align directly with the Safety Net request for proposal.
  • Available for services that are well-established and have been in place for at least three years.

Federally qualified health centers and look-alikes can apply for core support, which is  the working capital nonprofit organizations need to sustain their day-to-day operations. This funding supports ongoing operations and represents the maintenance or expansion of core functions of an organization. This funding category may cover day-to-day activities or ongoing expenses, such as: salaries, utilities, office supplies, technology/facility maintenance, technology purchases, and professional development.

All new grants awarded in 2020 will be 12-month grants.

Additional Eligibility Criteria:

Eligible Applicants Include:
- Not-for profit organizations
- Tax-exempt organizations as defined by IRS Code Sections 501(c)(1), 501(c)(6), or 501(c)(9). (If your organization has applied for 501(c)(3) status but has not yet received it, you will be required to use a fiscal agent.)
- Organizations that are incorporated in one of the 50 states
- Organizations that can provide a Federal Employer Identification Number (or, for government entities, an enabling status).
- Organizations that maintain ongoing operations, including staffing and programs, within Kansas City, Missouri, and/or Missouri or Kansas service areas.
- Organizations that are a state or local government agency.
- Organizations that are in compliance with local, state, and federal regulations related to non-discrimination, wage and hour laws, workplace safety, licensure, protection of confidential health care information, and all other laws and regulations applicable to the staff, patients, consumers, and the workplace of the applicant
- Organizations that have the demonstrated capacity to carry out program work funded by the grant, exercise financial controls and use generally accepted accounting procedures.
- Organizations that have an annual audit performed by an independent CPA. Organizations that do not have a recent audit and IRS 990 will be required to use a fiscal agent.
- Religious organizations must provide non-evangelizing activities or programs that are available to the general public, and that are consistent with Health Forward principles of cultural diversity and cultural competence.

Note: In 2020, applicants for core support funding are limited to federally qualified health centers and health center look-alikes. Eligible agencies provide direct medical and dental services, work to accomplish missions that directly align with Health Forward’s mission, have a history of successful funding with Health Forward, and are subject to federal oversight.

The following is not funded:
- Individuals, for any reason, including paying for medical procedures, prescriptions, health insurance, education or training.
- Basic or applied biomedical research, biotechnology, or laboratory studies
- Endowments or contributions to permanent funds or accounts that are intended to yield interest or dividend-bearing income for the support of staff, programs, services or other operations
- Construction, purchase, non-mandated renovation, demolition, or re-purposing of any physical facilities or real property
- Supplanting or offsetting the maintenance of effort by government or public entities
- Supplanting private funding
- Political campaigns to support or oppose candidates for public office
- Evangelizing activities of religious organizations
- Payment of bonds, loans, notes, or any other form of sponsorship or outstanding debts of an applicant or its affiliate(s)
- Annual fundraising appeals

In 2020, Health Forward will be accepting streamlined applications from grantees with an approved multi-year plan in 2019 as well as new proposals. Streamlined applications will require a letter of intent and an abbreviated proposal. New proposals will require a letter of intent and a full proposal.

All applicants, including returning applicants who had established accounts in the former grantee portal, must complete an eligibility quiz and register in the new system. Anyone wanting to register for the grantee portal should contact the email listed below. Your account in the new system will be activated and you will be provided with instructions to login.

Health Forward invites prospective applicants to visit the office to review exemplary proposals from previous funding rounds. Applicants may request feedback from the program officer(s) who reviewed their proposal. Feedback sessions may be conducted in person or over the phone within six months of receiving the award or the declination.

Health Forward offers technical assistance to nonprofit organizations with annual budgets under $500,000 for both applicant defined grant and foundation defined grant requests. Assistance includes up to eight hours of contracted grant writing support funded by Health Forward.
